ASQ | 2011 Northern Illinois Quality Conference

 

 

We are very pleased to announce the availability of the on line registration and the payment system. We do not store your credit card information in our system. Your payment information is securely processed by Paypal, Inc. You can also register by downloading the following registration form (in Microsoft Word format). Pre-register by November 5, 2011 and pay only $125 for the conference. Cancellations made before November 5, 2011 will be charged a $25 processing fee.  No refunds after November 5, 2011. Substitutions are allowed.
 

Instructions

Register for the conference
  1. Complete all of the information in the top of the form below, select either "No Workshops" or "At least 1 workshop" and click on the "Add to Cart" button. You will be taken to a PayPal page that shows the content of your cart.
  2. If you are not registering anyone else and not registering for workshops, click on "Proceed to Checkout", otherwise, click on "Continue Shopping". You will be taken either back to the registration page, where you can register additional people for the conference or register for workshops, or to the PayPal checkout page (see below).
  3. Repeat #1 and #2 above for any additional people that you want to register for the conference. They will be added as separate lines on your cart.

Register for workshops
  1. Enter the name of the workshop attendee in the appropriate space and click on the "Add to Cart" button. You will be taken to a PayPal page that shows the content of your cart.
  2. If you are finished with all registrations, click on "Proceed to Checkout", otherwise, click on "Continue Shopping". You will be taken either back to the registration page, where you can register additional people for the conference or register for workshops, or to the PayPal checkout page (see below).
  3. Repeat #1 and #2 above for any additional workshop registrations, entering the name of the attendee for each. They will be added as separate lines on your cart.

Check Out
When you have completed all of the registrations, please double check the items in your cart to make sure that all of the registrations, both for the conference and the workshops, are correct. If you need to make changes, you can remove the bad items from the cart and return to the registration page to enter the correct information. Click on "Proceed to Checkout". You will be taken to a PayPal page where you will be asked for credit card information. Follow the instructions to complete your payment. You will receive a verification email showing all of your registrations.

At any time you can click the "View Cart" button at the bottom of the page to see what registrations you have completed.
